Thankyousomuch,allofyou,forjoiningustoday. AndIwanttothankJulie,Barbara,Roland,Stephen,Renee,andChristopher,standing1behindme--physicians,physiciansassistants,andnurseswhounderstandhowimportantitisforustomakemuchneededchangesinourhealthcaresystem.
Iwanttothankallofyouwhoareheretoday. Iwanttospecially2recognizetwopeoplewhohavebeenworkingtirelesslyonthat--onthiseffort,mySecretaryofHealthandHumanServices,KathleenSebelius--(applause)--aswellasourquarterbackforhealthreformoutoftheWhiteHouse,Nancy-AnnDeParle. (Applause.)
WebeganourpushtoreformhealthinsurancelastMarch,inthisroom,withdoctorsandnurseswhoknowthesystembest. Andsoit’sfittingtobejoinedbyallofyouaswebringthisjourneytoaclose.
LastThursday,IspentsevenhoursatasummitwhereDemocrats3andRepublicansengagedinapublicandverysubstantive4(大量的,实质的)discussionabouthealthcare. Thismeetingcappedoffadebatethatbeganwithasimilarsummitnearlyoneyearago. Andsincethen,everyideahasbeenputonthetable. Everyargumenthasbeenmade. Everythingthereistosayabouthealthcarehasbeensaid--(laughter)--andjustabouteverybodyhassaidit. (Laughter.) Sonowisthetimetomakeadecisionabouthowtofinallyreformhealthcaresothatitworks,notjustfortheinsurancecompanies,butforAmerica’sfamiliesandAmerica’sbusinesses.
Now,wherebothsidessaytheyagreeisthatthestatusquo(现状)isnotworkingfortheAmericanpeople. Healthinsuranceisbecomingmoreexpensivebytheday. Familiescan’taffordit. Businessescan’taffordit. Thefederalgovernmentcan’taffordit. Smallerbusinessesandindividualswhodon’tgetcoverage5atworkaresqueezedespeciallyhard. Andinsurancecompaniesfreelyration6(配给,定量供应)healthcarebasedonwho’ssickandwho’shealthy;whocanpayandwhocan’t. That'sthestatusquo. That'sthesystemwehaverightnow.
DemocratsandRepublicansagreethatthisisaseriousproblemforAmerica. Andweagreethatifwedonothing-–ifwethrowupourhandsandwalkaway-–it’saproblemthatwillonlygrowworse. Nobodydisputesthat. MoreAmericanswilllosetheirfamily'shealthinsuranceiftheyswitchjobsorlosetheirjob. Moresmallbusinesseswillbeforcedtochoosebetweenhealthcareandhiring. Moreinsurancecompanieswilldenypeoplecoveragewhohavepreexistingconditions,orthey'lldroppeople'scoveragewhentheygetsickandneeditmost. AndtherisingcostofMedicareandMedicaidwillsinkourgovernmentdeeperanddeeperanddeeperintodebt. Onallofthisweagree.
Sothequestionis,whatdowedoaboutit?
Ononeendofthespectrum7,therearesomewho'vesuggestedscrapping8oursystemofprivateinsuranceandreplacingitwithagovernment-runhealthcaresystem. Andthoughmanyothercountrieshavesuchasystem,inAmericaitwouldbeneitherpracticalnorrealistic.
Ontheotherendofthespectrum,therearethose,andthisincludesmostRepublicansinCongress,whobelievetheansweristoloosenregulationsontheinsuranceindustry--whetherit'sstateconsumerprotectionsorminimumstandardsforthekindofinsurancetheycansell. Theargumentis,isthatthatwillsomehowlowercosts. Idisagreewiththatapproach. I'mconcernedthatthiswouldonlygivetheinsuranceindustryevenfreerrein9(驾驭,控制)toraisepremiums10anddenycare.
SoIdon'tbelieveweshouldgivegovernmentbureaucrats11(官僚)orinsurancecompanybureaucratsmorecontroloverhealthcareinAmerica. Ibelieveit'stimetogivetheAmericanpeoplemorecontrolovertheirhealthcareandtheirhealthinsurance. Idon'tbelievewecanaffordtoleavelife-and-deathdecisionsabouthealthcaretothediscretion12ofinsurancecompanyexecutivesalone. Ibelievethatdoctorsandnursesandphysicianassistantsliketheonesinthisroomshouldbefreetodecidewhat'sbestfortheirpatients. (Applause.)
Now,theproposalIputforwardgivesAmericansmorecontrolovertheirhealthinsuranceandtheirhealthcarebyholdinginsurancecompaniesmoreaccountable. ItbuildsonthecurrentsystemwheremostAmericansgettheirhealthinsurancefromtheiremployer. Ifyoulikeyourplan,youcankeepyourplan. Ifyoulikeyourdoctor,youcankeepyourdoctor. Icantellyouasthefatheroftwoyounggirls,Iwouldnotwantanyplanthat#p#分页标题#e#interfereswith(干扰,妨碍)therelationshipbetweenafamilyandtheirdoctor.
Essentially13,myproposalwouldchangethreethingsaboutthecurrenthealthcaresystem. First,itwouldendtheworstpracticesofinsurancecompanies. Nolongerwouldtheybeabletodenyyourcoveragebecauseofapreexistingcondition. Nolongerwouldtheybeabletodropyourcoveragebecauseyougotsick. Nolongerwouldtheybeabletoforceyoutopayunlimited14amountsofmoneyoutofyourownpocket. Nolongerwouldtheybeabletoarbitrarily(武断地,专横地)andmassivelyraisepremiumslikeAnthem15BlueCrossrecentlytriedtodoinCalifornia--upto39percentincreasesinoneyearintheindividualmarket. Thosepracticeswouldend.
Second,myproposalwouldgiveuninsuredindividualsandsmallbusinessownersthesamekindofchoiceofprivatehealthinsurancethatmembersofCongressgetforthemselves--becauseifit’sgoodenoughformembersofCongress,it’sgoodenoughforthepeoplewhopaytheirsalaries. (Applause.)
Thereasonfederalemployeesgetagooddealonhealthinsuranceisthatweallparticipateinaninsurancemarketwhereinsurancecompaniesgivebettercoverageandbetterrates,becausetheygetmorecustomers. It'sanideathatmanyRepublicanshaveembracedinthepast,beforepoliticsintruded16.
Andmyproposalsaysthatifyoustillcan’taffordtheinsuranceinthisnewmarketplace,eventhoughit'sgoingtoprovidebetterdealsforpeoplethantheycangetrightnowintheindividualmarketplace,thenwe'llofferyoutaxcreditstodoso--taxcreditsthatadduptothelargestmiddle-classtaxcutforhealthcareinhistory. Afterall,thewealthiestamonguscanalreadybuythebestinsurancethereis,andtheleastwelloffareabletogetcoveragethroughMedicaid. Soit'sthemiddleclassthatgetssqueezed(挤,勒索),andthat’swhowehavetohelp.
Now,itisabsolutelytruethatallofthiswillcostsomemoney--about$100billionperyear. Butmostofthiscomesfromthenearly$2trillionayearthatAmericaalreadyspendsonhealthcare--butalotofitisnotspentwisely. Alotofthatmoneyisbeingwastedorspentbadly. Sowithinthisplan,we’regoingtomakesurethedollarswespendgotowardsmakinginsurancemoreaffordable17andmoresecure. We’regoingtoeliminatewasteful18taxpayer19subsidies20(津贴,补助金)thatcurrentlygotoinsuranceandpharmaceutical21(制药的)companies;setanewfeeoninsurancecompaniesthatstandtogainalotofmoneyandalotofprofitsasmillionsofAmericansareabletobuyinsurance;andwe'regoingtomakesurethatthewealthiestAmericanspaytheirfairshareonMedicare.
Thebottomlineisourproposalispaidfor. Andallthenewmoneygeneratedinthisplangoesbacktosmallbusinessesandmiddle-classfamilieswhocan'taffordhealthinsurance. Itwouldalsolowerprescription22drugpricesforseniors. AnditwouldhelptrainnewdoctorsandnursesandphysicianassistantstoprovidecareforAmericanfamilies.
Finally,myproposalwouldbringdownthecostofhealthcareformillions--families,businesses,andthefederalgovernment. Wehavenowincorporatedmostoftheseriousideasfromacrossthepoliticalspectrumabouthowtocontaintherisingcostofhealthcare---ideasthatgoafterthewasteandabuseinoursystem,especiallyinprogramslikeMedicare. ButwedothiswhileprotectingMedicarebenefits,andextendingthefinancialstabilityoftheprogrambynearlyadecade.
Ourcost-cuttingmeasuresmirrormostoftheproposalsinthecurrentSenatebill,whichreducesmostpeople'spremiumsandbringsdownourdeficit23byuptoatrilliondollarsoverthenexttwodecades--bringsdownourdeficit. Thosearen'tmynumbers;thosearethesavings24determined25bytheCongressionalBudgetOffice,whichistheWashingtonacronym26(首字母缩略词)forthenonpartisan(无党派的),independentreferee27(裁判员,调解人)ofCongressintermsofhowmuchstuffcosts.(Laughter.)
Sothat'sourproposal. Thisiswherewe'veendedup. It'sanapproachthathasbeendebatedandchangedandIbelieveimprovedoverthelastyear. ItincorporatesthebestideasfromDemocratsandRepublicans---includingsomeoftheideasthatRepublicansofferedduringthehealthcaresummit,likefundingstategrantsonmedicalmalpractice(玩忽职守,不法行为)reform,andcurbing28wasteandfraudandabuseinthehealthcaresystem. Myproposalalsogetsridofmanyoftheprovisionsthathadnoplaceinhealthcarereform--provisionsthatweremoreaboutwinningindividualvotesinCongressthanimprovinghealthcareforallAmericans.#p#分页标题#e#
Now,despiteallthatweagreeonandalltheRepublicanideaswe'veincorporated,many--probablymost--RepublicansinCongressjusthaveafundamentaldisagreementoverwhetherweshouldhavemoreorlessoversight29ofinsurancecompanies. Andiftheytrulybelievethatlessregulationwouldleadtohigherquality,moreaffordablehealthinsurance,thentheyshouldvoteagainsttheproposalI'veputforward.
